A good accident/incident investigation process includes identifying the investigation team, establishing/reviewing written procedures, identifying root causes, and tracking all safety hazards found to completion. • The observations of co-workers can be critical, and if you are interviewed, heed the following:
• Make mental or written notes about the accident before the investigation starts.
• Avoid talking to others before talking to the investigator, since this may confuse the facts.
• Answer all questions about the incident as accurately as possible.
• Don’t change anything at the scene of the accident.
